It's Adol from Ys VI; the Ark of Napishtim, fighting the very first boss...! This really happens if you forget to dodge it. I even died this way the first time I fought the boss... It's even worse if you've finally defeated him on Nightmare mode after endless attempts, and then to die like this is... Well... You get the idea.
